Job Summary
The Licensed Vocational Nurse – Home Care (LVN_HC) performs patient care services that supports the Primary Care Clinic, Adult Day Health Center (ADHC Floor) and Home Care under the clinical supervision of the Primary Care Provider (PCP), Nurse Practitioner (NP) or other licensed staff. The LVN HC maintains a clean, organized, and safe environment. Ability to handle a diverse range of situations and responsibilities is the primary duty of an LVN HC Including but not limited to the planning and monitoring of in-home services and for carrying out tasks in the participants’ residences as assigned such as home safety assessments, data gathering and supervisory visits.

Essential Job Functions:

Duties include but are not limited to:
  • Participate in IDT and Care Plan Meetings as scheduled.
  • Collaborate with HCC LVN, HC Lead/Personal Care Assistant supervisor, to ensure that participants are allocated enough patient care hours and services based on assessments.
  • Must be able to communicate effectively with participants/family observing IIH core values.
  • Enter and concurrently acknowledge pertinent patient information, results and collected data into electronic health record system
  • Must work within the LVN scope of practice in California, such as providing basic nursing care, administering medications, performing wound management, and assisting with basic assessment, all under the supervision of an RN and Physician.
  • Must be able to complete a Home Care assessment as scheduled per policy guidelines during pre-enrollment encounter, semi-annual, unscheduled and annual assessments
  • Follow workflow process upon completion of Home Care assessments.
  • Involved in the development and implementation of care plans.
  • Must be able to complete all assessments assignment(s) and other tasks in a timely manner prior to deadline
  • Document patient concerns, change of condition (COC) and any changes in participants’ baseline status and report to PCP/RN or appropriate staff as they arise.
  • Review, Follow up and Document any follow ups and Service Delivery Requests in a timely manner per policy guidelines.
  • Must be able to assist patients with rehabilitative exercises and activities.
  • Ensuring patients and their families understand discharge or after visit instructions
  • Ensure proper body mechanics and patients’ safety with ambulating/transfer, etc.
  • Must show competency on providing basic emergency care and first aid and use of AED as needed
  • Attend and participate in staff meetings, in-services, projects, and committees as assigned.
  • Adhere to and support the center’s practices, procedures, and policies including assigned break times and attendance
  • Accept assigned duties in a cooperative manner; and perform all other related duties as assigned.
  • The LVN HC may also be asked to perform other clerical duties as needed and requested to support daily clinic operation goals, benchmarks and quality patient care initiatives per department guidelines.
  • Be flexible in schedule of hours worked based on department and facility needs this includes providing coverage to all other Innovative Integrated Health facilities
  • May require use of personal vehicle.
Duties during ADHC Floor assignment:
  • Assist in processing participant’s needs via SBARs received through email, phone call or in person.
  • Administer medications and provide education to patient/family under supervision of the PCP/RN of the medication name, quantity on hand, medication use and any side effects or adverse reaction as prescribed on the written prescription or printed medication list.
  • Administer Center Administered Medications – oral, topical, injectable medication(s) including insulin, immunizations within an LVN HC scope of practice and as prescribed and directed by the PCP
  • Administering IV therapy is permitted for LVNs with a certificate from a Board-approved training provider.
  • Perform accurate vital sign measurements such as blood pressure, temperature, O2 saturation and weight.
  • Basic wound care includes cleaning and bandaging injured areas.
  • Monitor and reconcile All Clinic medication(s) for outdates/expiration, quantity on hand and responsible for notifying Primary Clinic Manager or Nursing Supervisor of any changes or ordering purposes.
  • Monitor and keep an up-to-date records/inventories of the crash cart and its contents.

  • Ability to access all areas of the center throughout the workday.
  • Ability to lift up to 35 pounds occasionally, 15 pounds frequently, and 7 pounds constantly; required to obtain assistance of another qualified employee when attempting to lift or transfer objects over 25 pounds.
  • Requires constant hand grasp and finger dexterity; frequent sitting, standing, walking and repetitive leg and arm movements, occasional bending, reaching forward and overhead; squatting and kneeling.
  • Ability to communicate verbally with an excellent comprehension of the English language.
  • Majority of the work is generally performed outside of the facility in participant’s home also called field work.
  • Minimum work performed onsite including completion of documentation, providing facility coverage and other assigned tasks as a floor nurse.
Experience
  • Minimum of one (1) year of demonstrated successful experience in the Quality healthcare field of work
  • Minimum of one (1) year of documented experience working with a frail or elderly population.
Education and Certification
  • Active LVN license in the state of California
  • IV certification preferred
  • CPR certification
  • Is medically cleared for communicable diseases and has all immunizations up-to-date before engaging in direct participant contact.
