A busy and growing home care company in Tempe is looking for a Client Care Manager to add to our Client Services Team. This position is perfect for someone who enjoys a challenge and is good with people over the phone. Our Care Managers match our wonderful Caregivers with our wonderful Clients. If you are looking to make a difference in the life of a senior by finding them a Caregiver that helps them stay in their home, then this is the job for you! The perfect candidate has scheduling/staffing experience in a home care environment as well as Caregiving experience.

Duties:
Maintain relationships with our clients, their families, and caregivers to ensure they are getting the best care we can provide for them including:
–Schedule the appropriate Caregiver with the appropriate Client
-Read and interpret plans of care
-Update clients
-Review plans of care with Caregivers
-Speak with Client’s family members about concerns and/or schedule changes
-Speak with Case Managers about concerns and/or schedule changes
-Read interpret and enter authorizations
-Entering schedules
Work schedule:
Mon- Fri 8a – 5 p
*additional time spent to ensure client coverage
*on call one week day 5 p – 8 am
Job Type: Full-time
Pay: From $45,760.00 per year
