General:
The Occupational Therapist provides occupational therapy services to patients including evaluations, planning, and individual treatment programs and goals, direct patient treatments, discharge planning, and other related duties as required.
Qualifications:
- Graduate of an accredited program of Occupational Therapy
- Valid Occupational Therapy license in the state where services are rendered.
- Comply with Federal and state occupational therapy regulations
Physical, Mental and Sensory Requirements: Must possess the ability to effectively communicate with therapy staff, patients and others as needed, as well as the ability to effectively respond to the environment within the long term care setting. Ability to lift/walk patients, provide active therapy, and stand or sit for long periods of time, all day, if necessary. Ability to read, speak and understand English. Must be able to safely assist a patient in transfers, bed mobility, ADL?s and ambulation.
Essential Job functions:
The following are essential job duties that must be safely with or without reasonable accommodation without posing a direct threat to other employees, patients/residents or self.
- Upon referral, performs an evaluation to determine the problems and goals of the patient in collaboration with the patient and physician.
- Develops a treatment program based on initial evaluation to achieve needs and goals and makes appropriate changes in the treatment program according to patient?s performance.
- Administers scheduled and supervises direct patient care utilizing appropriate occupational therapy techniques.
- Instructs patient and/or patient?s family in home program.
- Completes patient screens on admission from referral quarterly or annually to determine if OT evaluation is warranted.
- Maintains records pertinent to patient treatment.

- Weekly summary
- 30 day physician updates
- Discharge summaries
- Completion of Medical Information Forms
- Patient care plan
- Telephone orders
- Communicates with patient?s physician regarding changes or modifications in treatment plan or patient status.
- Participates in patient care rounds, rehab team conferences, conferences with physicians and members of the rehab team as necessary regarding progress, problems and specific needs of the patient.
- Involved in providing in-service education to all personnel as assigned.
- Studies current trends and new developments in occupational therapy by survey of current literature and attendance at professional meetings and conferences.
- Conduct department tours for interested individuals/groups.
- Keeps an informal tabulation of supplies on hand. Notifies Director of Occupational Therapy of equipment needs and/or requests.
- Other duties as assigned.

NON-ESSENTIAL JOB FUNCTIONS

Requirements:
Graduate of an accredited program of Occupational Therapy
Valid Occupational Therapy license in the state where services are rendered.
Ability to comply with Federal and state occupational therapy regulations.
Previous experience as an Occupational Therpaist in a LTC environment preferred.
Compliance with applicable health laws and regulations, including but not limited to those relating to COVID-19 and influenza vaccinations.
Ability to speak, read and write in English
Must be able to move intermmittently throughout the work day.
Must be able to cope with the mental and emotional stress of the position.
Must possess sight/hearing senses or use prothesis that will enable these senses to function adequately so that the requirements of this position can be fully met.
Must be in good general health and demonstrate emotional stability.
Must be able to lift and move residents, equipment, supplies, etc. in excess of fifty (50) pounds.
