Summary:
The Ambulatory Medication Access Coordinator -Certified will coordinate aspects of medication access, including but not limited to collaborating with physicians, pharmacists, and clinical staff to help patients overcome barriers to medication access while operating within clear protocols and guidance from Ballad Health Medical Associates. The coordinator must be able to provide services to meet the needs of pediatric, adolescent, adult and geriatric patient populations served at Ballad Health Medical Associates. Personal characteristics necessary for successful performance in this position include flexibility, attention to detail, initiative, cooperation, and a commitment to continuing education to upgrade skills in a constantly developing profession. This individual should demonstrate good communication and interactive skills, oral and written, a positive outcome/results orientation (including accountability for results), and the ability to function successfully as part of interdepartmental or multidisciplinary teams. The Ambulatory Medication Access Coordinator- Certified shall support Ballad Health’s Mission and Vision and shall be committed to the improvement of care quality.
Responsibilities will also include:
- Assists with recruitment, mentoring, and training of incoming Ambulatory Medication Access Coordinators.
- Provides regular education and/or training for Ambulatory Medication Access Coordinators and other clinical staff completing prior authorizations.
- Performs tasks related to prescription billing, including completing benefits investigations, completing prior authorizations, and locating prescription drug formularies.
- Navigates medication cost/access barriers by completing patient assistance applications, utilizing drug coupons, and/or assisting with low-income subsidy and dual eligibility applications.
- Communicates with patients regarding refill requests and other prescription questions from patients/pharmacies and forward messages and/or pending orders to providers for verification/authorization.
- Processes orders for continuous glucose monitors through durable medical equipment companies and submit to providers for verification/authorization.
- Ensures patient insurance information is up to date in the electronic health record.
- In collaboration with the Health Improvement Team, completes medication adherence calls and/or other population health tasks for medication-related quality measures.
- Remains compliant with State and Federal laws, regulations, clinical practice guidelines, and standard operating procedures.
- Remains actively engaged with professional development and continued education.
- Provides professional and collaborative service excellence to all patients and team members.
- Communicates with prescribers and pharmacists about drug shortages, recalls, and discontinuations.
- Gathers detailed medication history from patients and documents accordingly in an electronic health record with verification/authorization performed by provider and/or Clinical Pharmacist.
- Ensures adherence with BHMA controlled substance monitoring protocols (ensuring UDS, contracts, and pill counts are completed).
- Collects clinical monitoring information (i.e. labs, relevant patient history, etc.) for pharmacist review.
- Performs other office assistant tasks as needed, such as scheduling appointments, scanning, etc.
Requirements:
High School Diploma or Equivalent
Valid and active pharmacy technician license
National Certified Pharmacy Technician certification.
Preferred qualifications: Minimum five to eight years of experience in retail pharmacy, hospital pharmacy, or physician office setting.
1) Candidate has previous technician experience and already has competencies in above skills (if applicable). 2) OR candidate presents with previous specialized experience which is equivalent to a current service or program and meets an organizational need.
