Position Summary
We are seeking a compassionate, organized, and customer-focused Admissions & Medical Records Coordinator to join our skilled nursing team. This role serves as the first point of contact for prospective residents, families, hospitals, physicians, and community referral partners while ensuring the accuracy and compliance of resident medical records.
The ideal candidate is an excellent communicator with strong organizational skills, the ability to multitask in a fast-paced healthcare environment, and a commitment to providing exceptional customer service throughout the admissions process.
Primary Responsibilities
- Admissions & Referral Coordination
- Serve as the primary contact for admissions, referrals, facility tours, and prospective resident inquiries.
- Build and maintain strong relationships with hospitals, physicians, case managers, discharge planners, home health agencies, insurance providers, and community partners.
- Conduct clinical and financial pre-admission assessments to determine admission appropriateness.
- Coordinate all aspects of the admission process, including transportation, documentation, physician communication, and insurance authorization.
- Complete required admission documentation, including PASARR screenings.
- Verify insurance benefits and communicate with payers regarding authorizations.
- Collaborate with nursing, therapy, social services, pharmacy, and administration to ensure smooth resident transitions.
- Maintain accurate documentation of all referral and admission communications.
- Participate in census development and occupancy growth initiatives.
- Medical Records Management
- Maintain resident medical records in compliance with state and federal regulations.
- Assemble, organize, audit, and maintain resident charts.
- Process medical record requests.
- Print and maintain electronic health records as needed.
- Break down closed charts and retain records according to regulatory requirements.
- Update resident demographic information and face sheets.
- Maintain admission/discharge logs and daily census reports.
- Obtain physician documentation and ensure medical records are complete, accurate, and organized.
Team Collaboration
- Participate in interdisciplinary meetings, care conferences, quality assurance activities, and discharge planning.
- Support nursing staff with documentation and care coordination.
- Assist with rehospitalization prevention efforts and resident discharge planning.
Perform other duties as assigned.
Qualifications | Required:
- High school diploma or GED.
- Customer service experience in a healthcare or professional office setting.
- Strong organizational, communication, and interpersonal skills.
- Proficiency with Microsoft Office and general computer applications.
- Ability to manage multiple priorities while maintaining attention to detail.
Preferred
- Previous admissions experience in a skilled nursing facility, rehabilitation center, hospital, or healthcare setting.
- Medical office or medical records experience.
- Knowledge of Medicare, Medicaid, and insurance authorization processes.
- Familiarity with medical terminology.
- Knowledge of medical coding is a plus.

Physical Requirements
- Primarily an office-based position with visits throughout the facility and community.
- Ability to sit, stand, walk, bend, and lift up to 30 pounds occasionally.
- Ability to work in a healthcare environment following infection control and safety procedures.
- Ability to manage frequent interruptions in a fast-paced environment.
Requirements
- Arizona Fingerprint Clearance Card (or ability to obtain).
- CPR Certification (or ability to obtain).
- Annual TB testing and required health screenings.
- Completion of required annual in-service education.
- Ability to respond during emergency or disaster situations in accordance with facility policies.

Pay: $18.50 – $23.50 per hour
